Alabama splits its requirements by drug class. Non-controlled legend drugs need nothing beyond the medical license; controlled substances bring a permit and a per-location registration.
Non-controlled legend drugs
Physicians may dispense medications in-office to their patients in Alabama under their state medical licenses. No dispensing permit is required for non-controlled legend drugs, provided the practice follows applicable state and federal law on storage, labeling, and records.
Controlled substances
A permit is required to dispense controlled substances, pursuant to Board of Medical Examiners Rule 540-X-4-.05. Each location where medications are dispensed must be registered separately, so a practice adding a second office cannot rely on the registration covering the first.
Mid-level providers
Mid-level dispensing rules vary considerably from state to state. Alabama regulations do not specify whether Physician Assistants (PAs) and Certified Registered Nurse Practitioners (CRNPs) may dispense non-controlled legend drugs. Where a state is silent rather than permissive, the safer course is to confirm directly with the Board before delegating any dispensing task.
PDMP reporting
Practitioners who dispense Schedule II through V controlled substances must report to the Alabama Prescription Drug Monitoring Program. Reporting is daily, with submissions due at least once each day by 11:59 p.m.
Alabama also requires a zero report on any business day when no controlled substances were dispensed, so the reporting obligation does not pause on a quiet day. Data must be transmitted electronically in a format compliant with ASAP standards.
The rule reaches dispensing practitioners directly, not only pharmacies: licensed physicians, dentists, podiatrists, and optometrists who dispense controlled substances are named among the entities required to report.
