Iowa divides its requirements at the controlled-substance line: non-controlled dispensing needs nothing extra, while controls bring a state permit with a recurring fee.
Non-controlled legend drugs
Physicians in Iowa may dispense non-controlled legend drugs within their medical purview, so long as they meet all labeling requirements and follow all state and federal regulations. No separate permit or application is required for this tier, which lets a practice open a dispensary around common non-controlled medications without a new license.
Controlled substances
Physicians dispensing controlled substances must apply for a controlled substance registration permit and pay a fee, currently $90 every two years. More information on the controlled substance application may be found on the Iowa Board of Pharmacy website here.
Because the permit renews on a two-year cycle rather than indefinitely, it belongs on the same renewal calendar as your DEA registration and medical license.
PMP reporting
Iowa requires dispensers to submit prescription data by the next business day after dispensing or administration. The obligation applies to dispensing prescribers, not only to licensed pharmacies.
The reportable scope is worth checking against your formulary. Iowa’s program captures controlled substance prescriptions and also reaches opioid antagonists — any provider who dispenses or administers naloxone falls within the reporting requirement, even where the rest of the formulary is non-controlled.
Labeling and records
Iowa’s non-controlled tier has no registration checkpoint, so nothing in the state’s process will review your labeling or storage before you begin dispensing. Secure storage, patient-specific labeling, and complete records of drugs received and dispensed remain the practice’s responsibility from the first dispense.
