New Hampshire requires no dispensing permit at all, but it tiers controlled substance quantities by schedule in a way that shapes what controls are useful for.
Authority to dispense
Physicians may dispense medications to their patients under their state medical license with no dispensing permit required. There is no application, fee, or registration step before beginning.
Controlled substance quantity limits
Controlled substance dispenses are limited by schedule:
- Schedule II medications: a 48-hour supply
- Schedules III through V: a 7-day supply
The tiering matters when planning a formulary. A 48-hour Schedule II supply covers an immediate post-procedure need and little else. The 7-day allowance for Schedules III through V is more workable, and is enough to complete many short courses of therapy without a pharmacy visit.
Neither limit can be extended by obtaining an additional permit, so these quantities are the design constraint for a New Hampshire dispensary rather than a starting point.

PDMP reporting
New Hampshire law requires all dispensers authorized to dispense Schedule II through IV controlled substances within the state to register with the PDMP and to submit information for each prescription dispensed.
Dispensations must be reported by the close of the next business day following the date the prescription is dispensed. Where a dispenser has no transactions to report for the preceding period, a zero report must be filed.
Labeling and records
Because no permit application exists to prompt a review, nothing in the state’s process checks your storage or labeling before you start. Secure storage, patient-specific labeling, and complete records remain the practice’s responsibility from the first dispense.
